Eligibility Requirements
To qualify for a Kentucky Medical Cannabis Card, you must meet the following criteria:
Age & Residency
You must be a legal resident of Kentucky
You must be 18 years or older
Patients under 18 require a parent or legal guardian to provide written consent and serve as the designated caregiver
Qualifying Medical Conditions
Kentucky law requires that patients have a recognized medical condition such as:
Cancer (any type or stage)
Chronic or debilitating pain
Epilepsy or other seizure disorders
Multiple sclerosis, muscle spasms, or spasticity
Chronic nausea or cyclical vomiting syndrome resistant to conventional treatment
Post-Traumatic Stress Disorder (PTSD)
Other conditions as approved by the Kentucky Center for Cannabis
Medical Documentation
To help support your evaluation, please bring at least one of the following:
Doctor’s visit notes or clinical summaries
Hospital discharge paperwork
Referrals from specialists or chiropractors
Prescription history from a local pharmacy
Proof of prior medical cannabis certification (from another state)
