How to Apply With the Kentucky Office of Medical Cannabis

This guide is for patients who have already completed a face-to-face consultation with a KYMMC physician.

Next Steps After Your Appointment

Because KYMMC can't directly process state applications or access their systems, all final steps are handled exclusively through the Kentucky Office of Medical Cannabis. Contact them if you run into technical issues or trouble logging into your account.

1. Create or Log In to Your State Account

  • Visit the state portal at the specified login site.

  • Click “Register” if you're new, or log in with your existing credentials.

2. Initiate a New Patient Application

  • From your dashboard, choose “Create New Application.”

  • Select “New Patient Registration” and complete the online form.

3. Accept Your Certification

  • Navigate to the “Practitioner Written Certification Information” section.

  • Click “View Available Certifications” and select the one authored by your KYMMC provider.

4. Upload Your Notarized Form

  • Scan or photograph the notarized signature page.

  • Upload it to the designated section of the portal, usually the fifth tab.

5. Complete Your Background Check Payment

  • In the seventh tab, pay the required $25 fee.

  • Note: This payment is made to the state, not to KYMMC. For payment-related issues, contact the Kentucky Office of Medical Cannabis at (833) 727‑2828.
